What exactly does "clear product definition and requirements" mean?

Or conversely, what are the observed patterns of unclear product definition and requirements that are causing the uncertainty.

I say that because "the requirements aren't detailed enough" has been the lament of Engineering for decades and aside from iterative development practices, I've not seen any significantly sufficient format to define requirements more fully or completely.
